Welcome to Quick Labs Mobile ("QLM," "we," "our," or "us"). By accessing or using our services, including our website, mobile applications, and in-home diagnostic services, you ("Client," "you," or "your") agree to comply with and be bound by these Terms of Service ("Terms"). If you do not agree, you must not use our services.

2. Overview of Services

QLM provides mobile diagnostic services, including in-home blood draws, lab test coordination, and related health services. Our services are delivered by trained professionals at your preferred location. We do not directly provide laboratory analysis; samples are sent to accredited labs for processing.

3. Eligibility

To use our services, you must:

Be at least 18 years of age or have consent from a legal guardian. Provide a valid physician’s order when required for diagnostic tests. Supply accurate and complete personal and health information during booking and service use.

4. Scheduling and Appointments

Appointment Requests: Clients can request appointments through our website, mobile application, or by contacting our support team. Appointment confirmations are subject to availability and will be finalized by QLM.

Changes and Cancellations: Changes or cancellations must be requested at least 24 hours before the scheduled appointment time. Late cancellations or no-shows may result in a fee, as specified at the time of booking.

Delays and Rescheduling: QLM reserves the right to reschedule or delay appointments due to unforeseen circumstances, such as traffic, weather, or emergencies. Clients will be notified promptly.

5. Payment Terms

Payment Obligations: Payment for services is required at the time of booking unless otherwise specified. Clients are responsible for ensuring that payment details provided are accurate and complete.

Insurance: QLM does not bill insurance providers directly. Clients may request itemized receipts for reimbursement claims with their insurance provider.

Refunds: Refunds are issued at QLM’s discretion for services not rendered due to circumstances beyond the client’s control.

6. Client Responsibilities

Accurate Information: Clients must provide truthful and complete personal, health, and scheduling information.

Preparation for Appointments: Clients must follow all preparation instructions provided, such as fasting or hydration requirements.

Safe Environment: Clients are responsible for ensuring that the service location is safe, clean, and accessible.

7. Privacy and Data Protection

Privacy Commitment: QLM is committed to protecting your personal and health information in compliance with applicable privacy laws, including HIPAA.

Use of Information: By using our services, you consent to the collection, storage, and use of your personal information as described in our Privacy Policy.

8.Limitation of Liability

Scope of Liability: QLM’s liability is limited to the cost of services provided. We are not liable for laboratory results, delays caused by third-party labs, or any indirect, incidental, or consequential damages.

Exclusions: QLM is not responsible for issues caused by incomplete or inaccurate information provided by the client; or external factors such as traffic delays, weather, or emergencies.

9.Prohibited Use

Clients agree not to: Use our services for unlawful purposes or in violation of any laws. Harass, threaten, or abuse QLM employees or contractors. Resell or misrepresent QLM services.

10.Modifications to Services
Right to Modify: QLM reserves the right to modify or discontinue services, pricing, or policies at any time without prior notice.

Amendments to Terms: These Terms may be updated periodically. Continued use of our services constitutes acceptance of the updated Terms.

11. Amendment and Modification
